I'm an esthetician and I'm curious about the intake protocols different folks follow with their clients. Can you share what you discuss, what forms you use, what you have clients sign? Or if you don't do anything please share this as well. Also curious about how clients react to your intake. Thanks.

These are the client intake forms that we use. They are included with our Crystal-Free Microdermabrasion DVD which demonstrates how to do a client consultation before the skin care procedure. These forms are designed to be used in any skin care protocol, not just for microdermabrasion. It is important that you go over all the questions with your clients to make sure that the procedures you are performing are a good fit for your client.

Feel free to download these forms that are attached and use them in your practice. There are two client consultation forms, a Skin Care Evaluation Form and an Informed Consent and Release Form.  Aesthetic VideoSource owns the copyright to these forms so you can't sell them or publish them, but you can use them in your practice.

If you are a member here they provide several different consent and intake client evaluation forms that are very thorough. I mostly use these for my client intake.

I have every client do a medical type form I got on the ASCP website and then update it when they come back. I always ask if they have any concerns or if their health has changed.  I specifically ask about medication, surgery, and hospitalizations in the past few months. 

Despite all of that, yesterday I had a new client tell me (as she left), "Oh, make sure to disinfect everything real good.  I have Hepatitis C."
