Hi, I had a client yesterday that came to me specifically for extractions under her chin and on her neck. She had deep blackheads and very thick skin. I work at Massage Envy Spa so my back bar is limited to their Murad products. I used an enzyme exfoliater on her, then went in to extract, except it was almost impossible. Her skin was so thick, I hardly removed anything. So my question is this, what would you all do to treat a client like this? Unfortunately at ME I am limited, but I also booth rent. So if I were in my esty room I can branch out to peels and other tools on hand. Any suggestions please...

enzyme exfoliater sounds great..perhaps warming the skin with massage or steam..sometimes clients need several treatments..do you have glycolic or salicylic

Something many forget is that the facial massage can also be used to ease extractions. You can use the petrissage technique to help loosen and bring some of the congestion and blackheads to the surface. You would use a firmer pressure, like for a mans facial, and can also massage under steam as a double whammy. A simple solution of baking soda and water can be used to help soften the sebum.

Recommend series of at least three microdermabrasion treatments before performing extractions on this client. Another thought, have you found out what is her home skin care routine? Black heads on the chin? Something is wrong or she is tweezing the chin herself. In addition I am assuming her hands are constasntly touching and feeling to find a hair. Bad habbit that needs to be eliminated. I hope this helps

I just bought an ultrasonic skin scrubber. I love it. I am able to get a lot of extractions done on think keratinized skin I couldn't reach before.

I misread the post, I would definitely do a series of micro treatments, and advise her to use a glycol cleanser at home

I just received certification in oriental bodywork; I am now adding acupressure work to my treatments..Having an understanding of the the  meridians in the body, especially the ones that start and/or stop in the face has really allowed to better treat acne and other conditions

I went to imageskincare website but couldnt find the "image resurfacing mask.  Do they still make it?

It does work well. Its a 15 or 17% blend of, I believe, salicylic, lactic, and glycolic (I might have this wrong, though, I'm tired) lol

If you are looking at limited resources to help the client (because they are a subscription member of ME), I would do the following:

-Cleanse with oil-based cleanser

-Cleanse with gel-based cleanser

-Use a microdermabrasion creme, if available

-Apply an enzyme mask, with steam towel application

-Apply moisture gel/serum

-Apply massage oil, massage in for several minutes

-Remove oil with steam towel

-Perform extractions......keep skin hydrated as you move about the face (distilled water and/or serum)

-Cleanse again with gel-based cleanser

-Toner of choice

-Hydration gel/serum

-Sunscreen (zinc-based)

I have found this technique to be highly efficient in the removal of deeply embedded comedones, when other options aren't available (either by state prohibition, or license restriction).

I am by no means an acne expert, but have found this to work very well for my clients who suffer from it.
